Create Windows XP style Notebook Icon
Create a rectangle using rectangular shape tool.

Press Ctrl+T. Right Click and select distort. Adjust the nodes to get the shape as shown.

Double click the layer to open layer style window. Apply settings as shown.

Duplicate, scale down and place as shown.

Ctrl+Click to get the selection. Contract the selection by 2 pixels. Create a new layer. Apply Black to white gradient. Change layer mode to overlay for this layer.
Create a rectangular shape as shown using pen tool.

Ctrl+Click to get the selection. Contract the selection by 2 pixels. Create a new layer. Apply Black to white gradient. Change layer mode to screen for this layer.
Create a rectangle for the pencil using rectangular shape tool.
Double click the layer to open layer style window. Apply settings as shown.
Create a rectangular selection using rectangular marquee tool. Create a new layer. Apply Black to white gradient. Change layer mode to screen for this layer.

 
Create a triangular shape using polygon shape tool. Apply gradient overlay layer style.
Create two rectangles for the upper part of the pencil. Apply gradient overlay layer style. Create 1 pixel line for the tip of the pencil.
Select and merge all the pencil layers. Rotate and place as shown.

Duplicate layer. Enable lock transparent pixels from layer palette. Fill it with black color. Press Ctrl+T. Right click the layer and select distort. Adjust nodes to create a perspective shadow. Reduce opacity to 50%.
