| Create a rectangle using rectangular shape tool. |
| Press Ctrl+T. Right click the layer and select distort. Adjust nodes to get the shape as shown. |
| Enable add to shape area form the top bar. |

| Press Alt key and keep hitting the down arrow key to create 3d extrusion. Select the top most duplicate layer and hide it. |
| Select & merge all other duplicate layers. Double click the layer to open layer style window. Apply layer style with the settings shown. |

| Unhide the top most layer. Double click the layer to open layer style window. Apply layer style with the settings shown. |

| Duplicate the top most layer and scale down. Again repeat the above steps to create a 3d extrusion of the scaled down layer. |
| Select and merge duplicate layers. Press Ctrl+T. Right click the layer and select warp. Adjust nodes to get the shape as shown. |
| Double click the layer to open layer style window. Apply layer style with the settings shown. |

| Using pen tool create a shape as shown below. |
| Double click the layer to open layer style window. Apply layer style with the settings shown. |

| Duplicate layer. Press Ctrl+T. Right click the layer and select flip horizontal. Inverse the gradient angle in gradient overlay settings. |

| Enable add to shape area form the top bar. Create another rectangle using rectangular shape tool to create the holy cross. |
| Copy the layer style we used for the first shape and paste on this layer. |
