Start by opening an image of an egg.
Press Ctrl+T. Right Click and select Warp. Adjust the nodes to get the shape as shown.
Insert cutout of a kid's hand.

Select the egg layer. Create a random selection using polygon lasso tool. Press Ctrl+J to copy paste the selection in a new layer.
Double click the layer to open layer style window. Apply settings as shown.
Similarly create some cracks and apply the same layer style.
Create a selection as shown in the image below.

Select egg layer. Press Ctrl+J to copy paste the selection in a new layer. Move this layer above the hand layer in layer order in layer palette.

Double click the layer to open layer style window. Apply settings as shown.
Similarly create more such shapes and apply same layer style.
Now scatter some of these shapes outside the egg in direction of fist. Let them have random rotations to create a realistic effect.

 
Select eraser tool and erase unwanted area of the arm layer.
